Employee Notification of Proof of Medical Insurance
As of January 2025, the IRS no longer requires a 1095-C form (Employer-Provided Health Insurance Offer and Coverage) to be filed with your taxes. Due to this change, the 1095-C form will not automatically be mailed to employees. If you would like a copy of your 1095-C, please email Kim Svacha, Renee Yager or Wendy Thorp. They will be happy to provide the form upon request.
